The following TERMS OF SERVICE constitute an agreement between you and LA-DOCTOR and govern your use of this Web site. By using this Service you agree to and understand the following:

PHYSICIAN REFERRAL SERVICES:
(1) To submit a physician referral request for any plastic or cosmetic surgery procedure, patients must be at least 18 years of age. We cannot refer patients under the age of 18 to our Member physicians. All physician referral requests for patients under the age of 18 must be submitted on their behalf by the parent or legal guardian of that patient. In addition, physician referral request forms which are submitted for this type of request must clearly state (in the comments box text area) that the form has been submitted by the under-aged patient's parent or legal guardian. (2) Your submission of a physician referral request form to us via E-mail does not constitute a legal obligation for us to attempt to find you a physician for any purposes including consultations. Not all submissions can be serviced and there are no guarantees that physician referral requests sent to us can or will be serviced. If possible, submitted referrals will be forwarded to the physician(s) of our choice. LA-DOCTOR reserves the right to determine which physician referral request submissions will be considered and serviced. (3) When using our service, you agree to provide true, accurate, current and complete information about yourself as prompted by our submission form. (4) No advice or information obtained through the use of our service shall create any warranty. (5) You use our Referral Service at your own risk. You agree not to hold LA-DOCTOR responsible or liable for any injuries or medical malpractice(s) and or claims for remuneration associated with any form of damage, foreseen or unforseen incurred by your visit to, or contact with via any medium, any and all physicians or medical services contacted as a result of your use of our Referral Service.

ONLINE FINANCING
User agrees that User is at least 18 years of age and warrants that all information submitted is true. User authorizes LA-DOCTOR to submit financing application to the financing company of LA-DOCTOR's choice. User understands that LA-DOCTOR is not a financing company and has no control over whether or not an application is accepted or not. User understands that all applications are final. Because applications are forwarded to a third party (finance company) submitted applications may not and cannot be cancelled. Please note that the PATIENT FINANCING APPLICATION FORM is not administered by LA-DOCTOR. By using this service you allow LA-DOCTOR to transmit your financial and personal information to a third party of our choice in order to facilitate financing your surgery. By using this service you also agree that LA-DOCTOR will assume no responsibility and will not be held liable for the misuse of information by any parties to which this information is transmitted. By using this service, User understands that all information submitted may be shared with a third party and that some of the information about User or User accounts may be shared among LA-DOCTOR and its related affiliate companies and/or partners for marketing and/or administrative purposes.

INDEMNITY: LA-DOCTOR does not endorse any of the physicians, medications, products, services or treatments described, mentioned or discussed in any of the services, databases or pages accessible from within or from any LA-DOCTOR Internet service or publication. Furthermore, LA-DOCTOR makes no representations concerning the efficacy, appropriateness or suitability of any such products or treatments contained on any of our Web sites. In view of the possibility of human error or advances in medical knowledge, neither LA-DOCTOR, its staff nor any other party involved in providing the services, databases or pages accessible within or from any of our Web sites or any other LA-DOCTOR service or publication, warrant that the information contained therein is in every or any respect accurate or complete and are not responsible nor liable for any errors or omissions that may be found in such information or for the results obtained from the use of such information. You are encouraged to consult other sources and confirm the information contained in any of the services, databases or pages accessible within or from this website. If erroneous or otherwise inaccurate information is brought to our attention, a reasonable effort will be made to correct or delete it. INTELLECTUAL PROPERTY: Our database contains information about physicians in Los Angeles and surrounding areas. Our data, listings or other constituent elements may not be downloaded, republished, resold or duplicated, in whole or in part, for any purposes especially for the compilation of mailing lists. Use of our site's materials and database for any purposes other than personal research is strictly prohibited. By using our service you agree to the above terms. Copyright infringement damages begin at $250,000 USD. Parties incurring aforementioned infractions will be liable for at least that amount pending full investigation of damages.
